Event Registration Process
> STEP 1: Download and print the Event Registration Form
> Registration form has two pages: release of liability and health information
> Downloadable forms in both Word and PDF format are listed below.
> Type your child's information in the Word file and save it to your computer to make future registrations easier.
> STEP 2: Complete and submit both pages of the Event Registration Form
> Remember to include health insurance information, including the policy number.
> Submit via fax: 860.675.4689.
> Submit in person at a church program (Sunday School, Wednesday night, etc.).
> Submit by mail to Student Ministries / Valley Community Baptist Church / 590 West Avon Rd / Avon, CT 06001.
> STEP 3: Submit payment with the Event Registration Form
> Make checks payable to "Valley Community Baptist Church" or "VCBC".
> Contact Student Ministries at 860.673.6826 x103 if you require financial assistance.
>> Registration deadlines are final
